From one OB code to 13+.

On January 1, 2027, CPT deletes the global obstetric package. A routine birth becomes 13 to 18 separately coded services, each with its own level, documentation and payer rule.

Size your volume

How many maternity codes will your team assign in 2027?

Your inputs

CMS proposed 8 (ACOG average-risk schedule). The AMA RUC valued 12.

RUC delivery mix: 20% primary and 10% repeat cesarean.

How we model it. AMA RUC building blocks. 1.5 labor dates per laboring patient; planned repeat cesareans (one-third of cesareans) have no labor code; 3 postpartum services after vaginal birth, 5 after cesarean. Deliveries spread evenly across the year.

Your 2027 maternity coding volume

16,740 coded services under 2027 rules, up from 1,200 global lines under 2026 rules. 322 maternity codes to assign every week.

The payer patchwork

Payers don't agree on when the switch happens.

Published transition rules use different triggers. A mixed payer book will run several rules at once through 2027.

Trigger: CMS final rule

UnitedHealthcare Commercial

Uses CMS G-codes if CMS finalizes them as proposed. Otherwise CPT phases, with E/M + TH for antepartum visits.

Bulletin · Oct 2026

Trigger: date of service

BCBS Vermont

Antepartum E/M + TH from 9/1/2026. Global codes on or after 1/1/2027 deny.

Notice · Aug 2026

Trigger: estimated delivery date

MetroPlusHealth (NY Medicaid)

E/M + TH from 6/1/2026 when the EDD is on or after 1/1/2027. Earlier EDDs keep global codes.

Provider policy · 2026

Trigger: none announced

Cigna

“Not changing its current maternity billing guidance at this time.”

Policy R11

Published positions as of October 26, 2026. The playbook covers more payers. Confirm current policy before billing.

Quick answers

Common questions.

When do the global OB codes go away?

59400, 59510, 59610 and 59618 are deleted for dates of service on or after January 1, 2027, along with 13 related codes.

How are prenatal visits coded in 2027?

One E/M per encounter, chosen by medical decision making or time. Many payers require modifier TH.

What makes labor management straightforward?

All six AMA criteria on that date: singleton vertex, routine monitoring, no physician intervention on fetal monitoring, normal progression or routine induction, stable conditions, and no previous cesarean. Anything else is complex.

Will Medicare keep a global maternity code?

Undecided. CMS proposed 15 global-style G-codes. The final rule is expected in early November 2026. We will update the playbook within 72 hours.
